need help leaving test of sanity

SPELLHOLD I got to the trolls during the test of sanity part but no giant troll head left behind nor does the apparition appear. Simply put, I'm stuck. I've searched the forums for any related topics but came up empty. I have tried reloading a save when leaving Bodhis gauntlet but this doesn't help. What happened? Here is my weidu log if needed.

Another Windspear crash

After finishing the Firkragg quest I attempt to leave Windspear (direction doesn't matter) but game crashes with this message "assertion failed in InfScreen WorldMap.cpp at line number 2120. What can be done to fix this? I'm not a programmer so detailed directions would be a big help. Could this be another CtB problem?
