Okay, think I've figured this out!
The creature files "deriny01.cre" (Erinyes), "gorbat02.cre" (A specific creature file for the Erinyes that appears as part of Ka'rashur's group in the Maze level of Watcher's Keep), "gorwom02" (The Huntress, a named Erinyes that appears as the group guarding the final seal at the bottom of Watcher's Keep) and "psbaat03" ("Baatezu Stalker") have all been changed by Infinity Animations to use the Female Elf avatar with added Erephine's wings item from One Pixel Productions' "1pp: Attachable wings" file in their helmet slot.
The problem with this is that the first three creature files listed are armed with bows, and the readme for Attachable wings clearly states "In its current form this implementation is not perfect. There are a few limitations concerned with using them. These are:
· Elves only
· Do not mix with swooshy robes, shields, bows and weapons in your left hand "
So I guess this combination of items is what's causing the assertion error as the game craps out trying to render it.
I've managed to fix this by copying the creature files "deriny01.cre", "gorbat02.cre", "gorwom02" and "psbaat03" from the "backup" subfolder of the "infinityanimations" folder into the main "override" folder.

In Topic: 100% Wild Surge Problem

I've just hit this as well. Took a while to track the culprit down to the Troll's Whisper ring from Every Mod and Dog (as it's supposed to level drain as well when put on, but good ol' CHARNAME was immune by this point and all I was seeing was getting turned green in exchange for a monstrous bump in hitpoints).
As suggested by arawn http://forums.pocket...p?topic=23350.0 here (sheesh, back in December 2006), once you've ditched the ring, editing your Bhaalspawn's character record in Shadowkeeper by browsing to the "Affects" tab and deleting all instances of "Type" 0x00000118 gets rid of the permanent Wild Surges.
For the record, it was more frustrating than fun having all my Paladin's spells and abilities, including ones from items, usually auto-fail with no explanation as to why it was happening. Having the auto-Wild Surges occur only while the Troll's Whisper ring was being worn would perhaps be the more consistent approach.

Hit a few bugs on my current playthrough.
1) After entering the Unseeing Eye section of the sewers, where the Lich's crypt is, after vanquishing the blind beholder, Fade kept trying to initiate dialogue with CHARNAME, but only the Player Initiated Dialogue options would pop up. This repeated until leaving the map.
2) Following the conversation where CHARNAME accepts Renal Bloodscalp's offer to investigate Mae'Var, the "Letter of Introduction" item isn't placed in inventory, meaning the conversation option with Gorch isn't present to open the doors to the back of Mae'Var's guildhall.
3) After the conclusion of the conversation where the retrieved evidence regarding Mae'Var's overtures to the Night Masks is delivered to Renal, all of the Shadow Thieves immediately turn hostile, along with Fade.
