Demon = Tanar'ri
Devil = Baatezu
Fiend = Any Baatezu, Gehreleth, Tanar'ri, or Yugoloth (possibly includes more, depending on who you ask).

But, yeah, Tanar'ri out number Baatezu by alot. They also tend to be more powerful individually as well. The difference is in cohesion and organization. Tanar'ri are as chaotic as they come and spend a significant amount of time killing each other and have very little in the way of large scale tactics.

As for the inconsistency of the game, no one really is running the whole show. There are dozens of writers and they all do there own thing and don't necissarily have to follow all the precidents of what came before. 3rd edition (wich is just D&D by the way, the Advanced was droped) is a bit better in this respect, but I feel they went a bit too far.

The Balor in BG2's underdark could have been there of any number of reasons. Maybe he was drunk and fell in a hole, got burried, then some gnomes accidentally dug him up 20,000 years later. Or, it could have been any number of containment spells. Maybe the Balor wanted to wait there for some oen to dig it up. Most anything is possible.

If you like the BG tangent, then feel free to use it and ignore anything contradicting it. Everyone has thier own version of the game.

Concerning the Underdark Balor I assume the gnomes uncovered a hidden portal of the many scattered around. The darkness dragon which resided in Mithrall Hall in RA Salvatore's book: Streams of Silver got there through one of these portals.

It could have been a temporary portal. If it were a permanent portal, there would be far more than a single Balor. ;)

Some are in the 3.5 Fiend Folio.

EDIT: Manual of the Planes I think has some too (mezzoloth?).

I think it might be a breach of copyright to post information from game products on the boards; I know people aren't allowed to post stats etc on the Wizards of the Coast boards.
