Background:
I played the game several times but stopped until discovering the fixpack by sheer chance revived my interest. Among a set of quests in the area behind the Tenement of Shadows is one to undo the repairs by the dabus. There are two different types of repairs to perform and one of the clickable areas is wide enough that when you click the mouse pointer could be on one of two adjacent buildings. I remember from when I previously played that depending on how you proceed with the repairs there was two possible layouts for the area if you returned after going forward to the Lower Ward. One of the layouts had a clear path to a house with a NPC inside.
Question:
I've forgotten how to do this. Can anyone remember how to do this? I've checked every walkthrough I found online and no one even mentions this game feature. I would appreciate any help.

I've never heard of this before either.
In the saves from my last playthrough, if you travel back to the Alley via the world map, the area is in fact reconfigured. You appear at the door to the Tenement of thugs, and instead of being able to go west and then south, you can go right. A new building -is- accessible, just a bit to the right of the door to the Tenements, but it doesn't have any NPC in it, it just has an empty chest and dresser. Is this what you're referring to? Note: Looking in the area file, and the INI file, and the scripts, there -is no- other area adjacent to the Alley, nor is there any way that an NPC can be created in that building.
There is only one trigger and dialogue apiece for the two areas you're supposed to fix, so I don't think anything changes based on what you do with those.
I don't see any way to prevent or alter the reconfiguration of the area as described above once you have made it to the Lower Ward.

The area does leave you with the feeling that it was originally going to have more content, but I don't recall anything like that either.
@Qwinn:
As a side-note I think it would be a good idea to also apply the Explore City Areas tweak to the Alley of Lingering Sighs. It is an open part of the city after all, it doesn't have any enemies and it has a big part that is not accessible and always stays hidden behind the fog of war.
