Teaming up with Balthazar causes failure in Melissan dialogue

I actually have a long list of bugs that I've encountered so far which I intend to post when I've finished my evil run through, but this was the first game-halting one (at least until I reloaded and killed him).

I thought you could only team up with Balthazar if you were of the "good" persuasion?

If I choose to work with Balthazar, once I reach the Throne of Bhaal, Melissan does not intiate dialogue automatically. If I speak to her manually, it starts normally, but ends after the first response option. After that, she cannot be spoken to again and the game will not advance.
I've just encountered a similar problem after convincing Balthazar to cooperate with me against Melissan, except the first battle in the Abyssal Plane goes differently.
What happens is that Melissan talks a bit, then Irenicus teleports in and talks a bit more, and finally Bodhi teleports in and talks for awhile. She then zaps Imoen, who drops out of my party and turns into a hostile ravager-type creature. I then have to avoid killing Imoen while battling Irenicus, Bodhi and a hostile Planetar. Once they are dead, Imoen turns neutral and will rejoin my party if asked.
However, the problem is that Melissan never comes back to fight any more battles, nor does Van-C-Bin, Cryonax, the Demigod, nor any other opponents ever appear. Since I don't get to fight Mellissan any more, the final battles don't trigger. The only thing I've been able to obtain more opponents is to use Ctrl-J to jump my party to the three outlying pod areas and then activate the pools in them. That will cause a batch of tough opponents to appear, but after killing them, still no Melissan.
Have cleared out the cache folder, but that didn't help. Is there any way to force Melissan to respawn? This is my first time thru Throne of Bhaal and I'm 99% finished, but I'd really like to complete that last little bit.
Oh well, in the meantime, I'll try going back to an earlier savegame and this time I'll just kill Balthazar.
Rats and double-rats!!!!! Killing Balthazar doesn't work any better than cooperating with him. Still have the same problem with Melissan appearing only once and no more.
