I have had this problem off and on for years. It is apparently caused by containers and the search that needs to happen to check for specific items. The more containers, the longer the check. It gets particularly worse if you have Jaheira in the party.
My solution has been to limit myself to one container and keep the number of items in the container down.

I know, but this doesn't seem to be case with my game: I've already removed every NPC AND every item in my inventory (in other words, playing with a single naked character as in the beginning of BGI), and the jerkiness is still as bad...
EDIT: Hmmm... pretty funny... the other day, I endured stuttering for half an hour or so - after which it vanished, without a trace. SWEET
