The silver cord is a tiny artificial conduit that connects an astral traveller to his/her physical body. It only appears for people born on the Prime Material (so not Planars), and only if they're astrally projecting onto the Plane (or nonPlane, to be precise), as opposed to travelling there "physically."

If the cord is cut, the person dies. End of story. On the other hand, if the astral body is killed while the cord remains, the person awakes back in their body while a planar without a cord will die.

The silver swords of the Githyanki have a small chance of cutting the silver cord. The stronger silver swords are +5 vorpal weapons as well, which is the sword you are probably thinking of.
